Information For Patients

GreeneMD Staff PhotoIn order for a new patient to schedule an appointment with Dr. Greene, a consultation fee of $150 is required. This fee is collected to schedule your initial appointment with him. The fee can be applied to your injectable treatment with Dr. Greene on the same day as the consultation. If a patient is not a candidate or elects not to have treatment that day, the fee is applied toward the consultation appointment and cannot be transferred to other services or patients. Please keep in mind when scheduling treatment around important events, as bruising and/or swelling may last 2 to 4 weeks. Health insurance is not accepted at our office, and all payments must be received at the time of service. Deposits are valid for two years.

Dr. Greene's highly experienced Physician Assistant Holly Shipley, MMS, PA offers complimentary consultations. She has been working alongside Dr. Greene in his office since 2013.

Appointment Cancellation Policy:

We take pride in delivering quality and personalized care. Each and every appointment space is important. If it is necessary to cancel your scheduled appointment, we require that you call or email us at least 24 hours in advance, and calling early in the day is appreciated.

If a NEW patient does not provide a 24 hour notice of an appointment cancellation (or does not show up to their appointment), then their paid $150 consultation fee will be forfeited. A new consultation fee of $150 will be collected to schedule their next appointment. The new deposit can be applied to treatment with Dr. Greene on the same day of the appointment. If the patient is not a candidate or elects not to have treatment that day, then the fee will apply towards the consultation fee with Dr. Greene.

If an EXISTING patient fails to provide 24 hours notice or misses an appointment, then a $150 fee will be collected in order to reschedule the appointment. This can be applied to treatment if it is the first time the patient has missed or late canceled an appointment. If the patient fails to show up or provide a 24 hour notice again, then the $150 fee is forfeited and not transferable.

Scheduling Your Appointment:

You can submit an online request for us to contact you, or you can call us at 954-651-6600. We recommend calling our office so we can speak to you in depth about your appointment. One of our Patient Care Coordinators will be happy to answer any questions you may have and schedule your consultation. When scheduling treatment around important events, please keep in mind that bruising and/or swelling may last 2 to 4 weeks.

Upon scheduling an appointment, you will receive an email confirmation from our office. The email will include your appointment date and time, our office address, and our appointment policy. If you are a new patient, please arrive 15-20 minutes prior to your appointment time to complete paperwork and registration.

We will contact you a couple of days prior to your appointment to confirm your appointment. If we do not hear back from you 24 hours prior to the scheduled appointment time, your appointment may be subject to automatic cancellation. Please expect to be in our office for approximately 1-2 hours during your appointment.

About Your Visit

You will be greeted by one of our friendly Patient Care Coordinators, who will complete your registration. All patients are required to bring a valid photo ID for proper patient identification. A copy of our notice of privacy practices is also available, so please let us know if you wish to receive a copy. We value patient privacy, and schedule ample time in between patients for your convenience and discretion.

Once in the treatment room, you will first meet with one of our highly experienced Medical Assistants, who can answer any preliminary questions you may have. Many patients elect to begin numbing while waiting to see the provider.

After the consultation, the Medical Assistant will review the treatment plan, including the costs and financing options. Your visit will be in a comfortable, no pressure atmosphere. We support your personal choice in selecting a provider with whom you feel comfortable and confident.
